In this training, we will take a deeper dive into Phase 4 of Wraparound, the Transition Phase, and the role of the FFS/Family Partner. We will break down the activities in this phase and how those activities support the bigger process of Wraparound.
We will take a look into Family Support Specialist best practices for engagement and helping the team prepare for meeting a family where they are at.
